From April 2025, we are changing the way we recall patients for long term condition reviews and medication reviews. We will be recalling patients around their month of birth. This may mean that patients will be contacted earlier or later this year to bring all reviews in line with their birth month.

Please respond when invited and if you have multiple conditions, we will aim to review these in the fewest number of appointments possible.
